For first-time patients
At Azabu-Juban Eye Clinic, we provide the following information to ensure that even first-time patients can receive treatment with peace of mind.
About Reservations
Advance reservations are required for the following:
– First contact lens prescription (junior high school students and above)
– Prism eyeglass prescription
– Examination and prescription for strabismus and amblyopia
You can also make reservations for the following tests.
– Visual field test
– Color vision test
Priority will be given to those who make reservations.
We may not be able to accommodate you on the day without a reservation, so we would appreciate it if you could make a reservation by phone.
No reservations are required for consultations other than those listed above.
If you come directly to the clinic during opening hours, we will see you in order of reception.
What to bring when you visit
- Health insurance card/My Number card (Myna Health Insurance Card): Please be sure to bring it with you.
- Medical card: Please bring it with you if you have one.
- Medication book: If you are currently taking any medication, please bring it with you to make your treatment go more smoothly.
- Glasses or contact lenses: Please bring them with you if you are currently using them.
- Referral letter: If you have a referral letter from another medical institution, please bring it with you.
Treatment flow

- When you visit the clinic, please present your health insurance card, My Number card (My Number health insurance card), etc. at the reception desk.
- step02Medical interview
- We will ask you in detail about your symptoms and concerns.
- step03Examination
- If necessary, we will perform a vision test and intraocular pressure measurement.
- step04Examination
- The doctor will examine you based on the test results and explain your symptoms and treatment plan.
If necessary, we will perform additional tests.
- step05Payment and prescription
- After the examination, you will pay at the reception desk and be given a prescription if necessary.
Notes
- Consultation hours: Weekdays 10:00-12:30 AM, 2:30-5:00 PM
- We accept patients until 12:30 PM in the morning and until 5:00 PM in the afternoon.
- Also, on Tuesday afternoons, an ophthalmologist from the Saiseikai will be in charge, but please note that he cannot speak English.
- Closed on: Saturdays, Sundays, and public holidays
